Documentation Needed to Enroll Student
Parent/guardian photo ID
Proof of Identity such as Birth certificate, passport, Baptism record. We do not accept Social Security card or number
Immunization record
Withdrawal form from last school if applying from Aug to Dec
Additional Forms/Items
- Kindergarten/1st grade Oral Health form
- Kindergarten/1st grade Health Examination form
- 7th grade requires a T-dap booster, current immunization record must be uploaded at time of application.
Other Items to Consider
- IEP or 504 records from current school are helpful
- Copy of custody order or restraining order, if any
- Latest test scores from current school are preferred
- Annual Prescription form for any medicine carried or used at school
- Caregiver Affidavit for guardianship
Complete returning student form via ParentSquare or paper copy from office.
Additional Forms/Items
7th grade requires a T-dap booster, current immunization record must be provided.
